Referral Extension Request

If you need to request an extension for your referral or prior authorization, you can do so directly through our Patient Portal.

We’ve made it simple for you. Just log in and follow the steps below.

1) Log in to your Patient Portal account.

2) Go to Messages or Requests.

3) Choose Referral Extension or Medication Refill.

4) Follow the on-screen instructions and submit your request.
